Medical Records Request

Patients can request their medical records by using the Request Medical Records Form and requesting a Medical Release of Information (ROI) Form.

Patients can then return the form via email and a member of Rock Medicine’s Administration team will be in contact within 48 Business Hours.

Please note – Medical records cannot be sent via Email. All records are sent via Mail (United States Postal Service).
